無治療の糖尿病に対し血糖コントロール開始後にみられた眼病変化の1例

Changes in retinopathy in a diabetic patient after initiation of first-time treatment

要約　61歳男性が精査のために紹介され受診した。11年前に糖尿病を指摘されたが無治療であった。矯正視力は右0.9，左1.5で，右眼底に点状出血が1個あった。内科でインスリンによる糖尿病治療が開始され，HbA₁cが急速に低下したが，開始の2か月後にしみ状出血と軟性白斑が両眼に多発し，以後6週の間，さらに悪化した。内科での治療開始とほぼ同時にプロスタグランジンE₁製剤が整形外科で投与されていたことが判明し，これを中止したあと網膜出血は消退した。急速な血糖コントロールとプロスタグランジンE₁製剤が網膜症悪化の原因であると推定した。

Abstract.　A 61-year-old male presented with untreated diabetes mellitus diagnosed 11 years ago. His corrected visual acuity was 0.9 right and 1.5 left. A dot hemorrhage was present in the right fundus. Funduscopic findings were otherwise normal. His HbAl_clevel showed remarkable decrease following start of treatment with insulin. Both eyes developed blot hemorrhages and soft exudates 2 months later. The findings aggravated during the ensuing 6 weeks. He had been receiving prostaglandin E₁for lesions in the spine. The retinopathy improved soon after its discontinuation. It appeared that aggravation of retinopathy was due to too abrupt control of diabetes mellitus and simultaneous use of prostaglandin E₁.
