Purpose:To describe a case of late-onset fungal endophthalmitis following cataract surgery. Case:A 79-year-old female underwent bilateral cataract surgery. Her left eye was diagnosed with endophthalmitis 2 months after surgery. She was referred to us 17 months after surgery after 5 episodes of endophthalmitis. Findings:Her corrected visual acuity was 0.9 right and 0.3 left. Her left eye showed a white mass in the lens capsule. Vitreous opacity was present. The left eye was treated by removal of intraocular lens, vitrectomy, and irrigation with antibiotics-added solution. Cultured vitreous specimen showed Candida parapsilosis. Systemic treatment with antifungal agents induced rapid disappearance of vitreous opacity. Conclusion:Fungus infection may induce late-onset postoperative endophthalmitis.
