白内障手術後のCandida parapsilosisによる真菌性眼内炎の1例

A case of endophthalmitis by Candida parapsilosis following cataract surgery

　目的：白内障手術1年後に発症した真菌性眼内炎の1例の報告。症例：79歳女性が両眼に白内障手術を受けた。その2か月後に左眼眼内炎と診断され抗生物質の投与を受けた。以後軽快と再燃を繰り返し，術後17か月に5度目の再燃があり紹介され受診した。所見：矯正視力は右0.9，左0.3。左眼に毛様充血があり，水晶体囊内に白色塊があり，硝子体混濁が併発していた。眼内レンズを摘出し，硝子体を切除し，抗生物質を加えた液で灌流した。術中の硝子体検体から真菌Candida parapsilosisが検出され，抗真菌薬を投与し硝子体混濁が急速に消退した。結論：抗生物質に抵抗を示す遅発性の術後眼内炎の原因として，弱毒細菌とともに真菌にも留意する必要がある。

　Purpose：To describe a case of late-onset fungal endophthalmitis following cataract surgery. Case：A 79-year-old female underwent bilateral cataract surgery. Her left eye was diagnosed with endophthalmitis 2 months after surgery. She was referred to us 17 months after surgery after 5 episodes of endophthalmitis. Findings：Her corrected visual acuity was 0.9 right and 0.3 left. Her left eye showed a white mass in the lens capsule. Vitreous opacity was present. The left eye was treated by removal of intraocular lens,vitrectomy,and irrigation with antibiotics-added solution. Cultured vitreous specimen showed Candida parapsilosis. Systemic treatment with antifungal agents induced rapid disappearance of vitreous opacity. Conclusion：Fungus infection may induce late-onset postoperative endophthalmitis.
