Ahmed glaucoma valveを挿入した難治性緑内障の術後経過

Outcome of treatment with Ahmed glaucoma valve for refractory glaucoma

　難治性緑内障5眼にAhmed glaucoma valveを挿入した。対象は新生血管緑内障3眼，ぶどう膜炎に続発した緑内障1眼，外傷に続発した緑内障1眼である。4眼には複数回の線維柱帯切除術が行われていた。術前の平均眼圧は39.6±7.4mmHgであった。術後1年の時点で，2眼で眼圧が点眼1剤を使用して21mmHg以下であった。角膜内皮細胞数は術後1年で約50％に減少していた。Ahmed glaucoma valveを使用することで，難治性緑内障の眼圧をコントロールできる可能性が広がる。術後の角膜内皮細胞の減少が著しいので，安易な使用は慎むべきである。

　We treated 5 eyes of refractory glaucoma by inserting Ahmed glaucoma valve. The series comprised neovascular glaucoma 3 eyes,uveitis-associated glaucoma 1 eye,and posttraumatic glaucoma 1 eye. Multiple trabeculectomy had bee performed in 4 eyes. Preoperative intraocular pressure（IOP）averaged 39.4±7.4 mmHg. One year after surgery,2 eyes had IOP of 21 mmHg or below with one topical medication. The corneal endothelial population had decreased to less than 50％ one year after surgery. The findings show that use of Ahmed glaucoma valve may increase of the possibility of IOP control in refractile glaucoma. Due precaution is necessary in its use because of associated damage to the corneal endothelium.
