緑内障を合併した毛様体黒色細胞腫の2例

Two cases of ciliary body melanocytoma with secondary glaucoma

目的：緑内障を併発した毛様体黒色細胞腫2症例の報告。症例と経過：症例は32歳と43歳の男性で，片眼の眼圧上昇による霧視で発症した。前房，隅角，硝子体腔に多量の色素顆粒があり，超音波顕微鏡（ultrasound biomicroscope：UBM）で毛様体に限局する腫瘍が発見された。両者とも腫瘍の局所切除を行い，病理学的に黒色細胞腫と診断された。両症例とも術後しばらくは低眼圧で経過したが，その後眼圧が再上昇し，瀘過手術の追加で眼圧コントロールが得られた。結論：眼内に多量の色素散布がある色素性腫瘍では，黒色細胞腫の可能性がある。

Purpose：To report two cases of melanocytoma of ciliary body associated with glaucoma. Cases and Findings：Two males,aged 32 and 43 years respectively,presented with unilateral blurring of vision secondary to glaucoma. Numerous pigment granules were present in the aqueous humor,chamber angle,and vitreous body. Ultrasound biomicroscopy showed a tumor located in the ciliary body in both cases. Surgically excised tumors were diagnosed with melanocytoma by histopathological studies. The intraocular pressure（IOP）increased again after surgery. Control of IOP was obtained by filtering surgery in both cases. Conclusion：Widespread pigment dispersion may result from intraocular melanocytoma.
