白内障手術術前患者の結膜囊常在細菌叢の検討

Bacterial flora in the conjunctiva of patients prior to cataract surgery

某総合病院眼科で白内障手術が予定されている429症例から結膜擦過物を採取・培養し，常在細菌叢を検索した。男性144眼，女性285眼であり，年齢は33～98歳，平均77歳である。395眼から細菌が検出され，613株が同定された。内訳はCoagulase-negative Staphylococcus（301株），Corynebacterium（181株），Staphylococcus aureus（36株）などである。メチシリン抵抗性黄色ブドウ球菌（MRSA）は4例から分離され，これらにバンコマイシン，レボフロキサシン，クロラムフェニコールの術前点眼を行った。術後の結膜培養でMRSAは検出されず，術前点眼が有効であると思われた。

Conjunctival scraping was obtained from 429 patients prior to cataract sugery in a general hospital. Bacterial culture showed 613 organisms from 395 eyes. They included coagulase-negative Staphylococcus（301 strains）,Corynebacterium（181 strains）,and Staphylococcus aureus（36 strains）. Methicillin-resistant Staphylococcus aureus（MRSA）was isolated from 4 eyes. These 4 eyes received instillation of vancomycin,levofloxacin,and chloramphenicol prior to surgery. MRSA was negative in cultured conjunctival scrapings after surgery,suggesting the efficacy for this therapeutic modality.
