　胸椎に発生した特発性脊髄ヘルニアを2例経験した．2例とも2～3年の経過で脊髄障害が進行し，1例はBrown-Séquard症候群を呈していた．画像所見では脊髄の前方への偏位を認めていた．いずれも術中所見で二重硬膜を形成しており，その内層の欠損孔から脊髄が脱出していた．ヘルニア孔を拡大し，短期の術後経過は良好である．中高年で胸椎レベルでの緩徐進行性の脊髄障害を呈している場合には脊髄ヘルニアも念頭に置き，早期に診断・外科的治療を行うことが良好な成績を得るために重要と考えられた．

　We report two cases of idiopathic thoracic spinal cord herniation.　Both cases presented with slowly progressive myelopathy, and one case presented with Brown-Séquard syndrome.　Radiography revealed anterior displacement of the thoracic spinal cord.　Intraoperatively duplication of the dura mater was observed in both cases, and surgical enlargement of dural defect improved the patient neurological symptoms.
