腰椎後方内視鏡手術における閉鎖式ドレーン留置についての前向き調査

Prospective Study of the Closed Suction Drainage System in Posterior Lumbar Microendoscopic Surgery

　腰椎内視鏡手術後は，血腫予防として術後ドレーン留置が推奨されているものの，ドレーンに関係するトラブルをしばしば経験する．しかしその現状についての報告はない．そこで今回，腰椎内視鏡手術後のドレーン留置にまつわる諸問題やマイナーな愁訴なども含めた前向き研究を行った．腰椎内視鏡手術(MED)65例，従来のオープン手術(Open)45例につき無作為にシリコン製(S群)，あるいは塩化ビニル製(P群)チューブを割り当て，ドレーンチューブ抜去前後の腰部違和感，創部痛，腰痛，下肢痛のVAS値を測定するとともに，ドレーンチューブの留置不良例，ドレナージ不良例などの状況を調査した．MED(S群)31例，MED(P群)34例，Open(S群)20例，Open(P群)25例を検討した結果，内視鏡手術では従来法に比し，ドレーン留置不良や誤抜去が高率に認められた．またドレーンチューブそのものは創部の疼痛や違和感の原因になりうるが，早期離床を行う内視鏡手術後ではさらに腰痛や下肢痛の原因にもなりうることが示された．愁訴に影響すると思われたドレーンチューブの材質については，シリコン製と塩化ビニル製の間では統計学的有意差は認めなかった．

　We performed a prospective study to elucidate the efficacy and problems associated with the closed suction drainage system following posterior microendoscopic surgery.　Drain placement was evaluated by plain radiography.　Discomfort and pain around the puncture site for the drain tube, low back pain, and leg pain were assessed on a visual analogue scale (VAS) before and after drain removal.　Inappropriate placement of the suction drain was observed in 12% of the patients in the microendscopic discectomy (MED) group, but not in any of the patients in the open surgery group.　There was significant improvement in the VAS scores for wound pain, back discomfort, back pain, and leg pain after surgery.　Placement of the suction drain caused some minor complications.　Statistical analysis showed that the material from which the drain was made was unrelated to the occurrence of the minor postoperative complications.
