腓腹筋に発生した腫瘤型筋サルコイドーシスの1例

Nodular Muscular Sarcoidosis in the Gastrocnemius Muscle. Case Report

　腓腹筋に発生した腫瘤型筋サルコイドーシスの1例を報告する．症例は50歳の女性で，両下腿後面の腫瘤を主訴に来院した．MRIではサルコイドーシスに特徴的なdark star signとthree stripes signsを認めた．しかしながら，診察や諸検査では腫瘤以外には異常を認めなかった．生検で組織学的に非乾酪性類上皮肉芽腫とラングハンス型巨細胞を認め，筋サルコイドーシスの診断に至った．筋サルコイドーシスに関する文献的考察を行った．

　We report a case of nodular muscular sarcoidosis in the gastrocnemius muscle.　A 50-year-old woman complained of palpable nodules behind her both leg.　Magnetic resonance imaging showed dark star and three stripes signs, which is characteristic for sarcoidosis.　However, on examination no abnormalities were found except for the nodules.　Biopsy of the gastrocnemius muscle was performed, and histological examination showed non-caseating epithelioid cell granulomas and Langhans-type giant cells, and muscular sarcoidosis was diagnosed.　We review the medical literature of previously reported cases of muscular sarcoidosis.
