踵骨後滑液包に発生した滑膜骨軟骨腫症の1例

Synovial Chondromatosis of the Retrocalcaneal Bursa. Case Report

　踵骨後滑液包に発生した滑膜骨軟骨腫症の1例を経験したので報告する．症例は50歳の男性で主訴は足関節部腫瘤である．8年ほど前から足関節外果部後方の腫瘤に気づいていたが，疼痛や増大傾向がなかったため放置していた．歩行時に腫瘤部の疼痛を自覚するようになり近医を受診した．精査の結果，悪性腫瘍が疑われたため当科を紹介され受診した．単純X線写真側面像では足関節後方の石灰化を認め，腫瘤はMRI T1強調で低信号，T2強調で等信号と一部低信号が混在する所見を呈していた．滑膜肉腫などの悪性腫瘍を疑い手術を施行した．手術時，アキレス腱前方の白色被膜を切開すると，内腔から多数の白色で軟骨様の腫瘤が摘出された．病理結果は滑膜骨軟骨腫症であった．臨床および病理所見から，本症例はMilgramの3期で，踵骨後滑液包に生じた滑膜骨軟骨腫症と診断した．足関節後方部の石灰化病変では距骨後滑液包と滑膜骨軟骨腫症の存在を念頭に置くべきである．

　We report a case of synovial chondromatosis of the retrocalcaneal bursa. A 50-year-old man complaining of swelling in his left ankle joint was referred to our hospital. Plain radiographs revealed a swollen soft area with calcification at the left posterior ankle. The lesion was viewed with low intensity on T1-weighted images, and with heterogeneous high intensity on T2-weighted magnetic resonance (MR) images. We performed tumor resection. Histological examination of the surgical specimens indicated synovial chondromatosis. Based on clinical and histological findings, we diagnosed the tumor as a synovial chondromatosis of the retrocalcaneal bursa. This result suggests that synovial chondromatosis and retrocalcaneal bursa should be considered when plain radiographs show calcification at the posterior ankle.
