We report a case of fat embolism syndrome that developed in a child after sustaining trauma. An 11-year-old boy fractured his 5th lumbar vertebra, pelvis, left femur, and both ankles in a fall from the 7th floor of a building. He was initially treated conservatively for the fractures of the femur and both ankles in the hospital. Two days after the injury, we diagnosed fat embolism syndrome based on the presence of anemia, hypoxia, low platelet count, and a confused state in the absence of a head injury. We supported respiration with a mechanical ventilator and stabilized the femur with an external fixation system. His postoperative clinical course was excellent. Although according to the literature fat embolism syndrome is very rare in children, the possibility of this syndrome should be considered in patients who have sustained high-energy fractures.
