血栓溶解療法ならびに抗凝固療法中に発症した大腿神経麻痺の1例

Femoral Nerve Palsy during Thrombolytic and Anticoagulant Therapy : Case Report

　抄録：症例は73歳，男性．急性心筋梗塞のために血栓溶解療法に抗凝固療法を併用した翌日から左下肢の筋力低下が出現し，その6週間後に当科を受診した．大腿四頭筋筋力はMMTで0，大腿神経と外側大腿皮神経領域の知覚障害を認めた．MRIで確認できた左側腸骨筋の血腫が大腿神経と外側大腿皮神経両麻痺の原因と診断した．保存的に治療を行い，発症後1年でMMTは大腿四頭筋で4まで改善し，伏在神経領域の知覚はほぼ回復した．血栓溶解療法や抗凝固療法を行う場合に合併症の1つとして腸骨筋血腫による大腿神経麻痺発生の可能性を考慮すべきである．

　A 73-year-old male who was receiving intravenous thrombolytic and anticoagulant therapy for acute myocardiac infarction developed left femoral nerve palsy. MRI showed a hematoma in the left iliacus muscle. Seven months after its onset, the palsy had improved to some extent without surgical treatment. Femoral nerve palsy secondary to an iliacus hematoma should be included among the complications of thrombolytic or anticoagulant therapy.
