　われわれは橈骨遠位端骨折に対する背側プレート固定術後に発生した長母指屈筋腱損傷の1例を経験したので報告する．症例は，背側プレート固定術後6カ月に母指の屈曲障害が出現し，われわれは橈骨遠位端掌側縁からのスクリューの突出による長母指屈筋腱の損傷であると診断した．手術は，プレートを抜去し，長掌筋腱を用いた腱移植術を行った．解剖学的に，橈骨遠位掌側面の手関節縁は，骨膜が露出した屈筋腱の滑走層（gliding floor）である．したがってこの部位での内固定材の使用は，骨皮質への突出が最小限になるように気をつけるべきである．

　The present article documents a case involving a flexor tendon rupture caused by an implant for the internal fixation of a distal radius fracture. The rupture extended from the point of dorsal plate fixation to the distal radius. Six months after surgery, active thumb flexion was impossible. We think that the restrictive thumb flexion was caused by damage to the flexor pollicis longus (FPL) tendon arising from the protrusion of a screw from the palm edge of the distal radius. The plate was removed, and the FPL was reconstructed using a palmaris longs tendon graft. Anatomically, the distal palmar edge of the radius forms a gliding floor for the flexor tendon directly above the periosteum. Therefore, the protrusion of implant screws from the bone cortex must be restricted in this area.
