　頚椎症性筋萎縮症（CSA）の病態は前角または前根の障害とされているが，その定義は明確でない．私たちは顕微鏡下に展開される神経根囊部の病態に興味をもって片開き式椎弓形成術を行ってきたが，CSAの病態として神経根の圧迫が高率に存在することを経験してきた．今回，明らかな索路症状や根性疼痛を伴わないCSA 15例を対象にその所見を分析し，CSAの病態を考察した．圧迫の主因は上関節突起であり，その先端による前根の選択的圧迫を3例に認めた．これは見落としやすい病態と考えられ，術中は根囊部の詳細な観察と神経根に対する的確な除圧が必要である．

　Selective ventral root lesions or damage to the anterior horns have been proposed as the pathology of cervical spondylotic amyotrophy (CSA), but they have not been well understood. We have observed the pathology around the nerve root during microsurgical decompression, which shows that the selective compression of the ventral nerve root is not rare. In this study, we discussed the pathology of CSA on the basis of our microsurgical experiences.
