脊髄腫瘍(A群)25例および脊髄腫瘍以外(B群)55例に対し,術前後のMaudsley性格テスト(MPI)および術後満足度調査を行い,性格心理状態の推移を検討した.改善率は有意にA群で低かったにもかかわらず,満足度はA群で有意に高かった.術前MPI陽性例は有意にA群で多かったものの,術後MPIが正常化したものはA群で多く,術後陽性化したものはB群で多くみられた.脊髄腫瘍患者では術前の精神的加重が大きく,術後改善率は低くとも術後の精神的不安定からの解放により,高い術後満足感が得られるものと考えられる.
Prospective study was performed to evaluate the personality and psychological traits before and after spinal surgery. Consecutive 80 patients (25 spinal tumors and 55 non-spinal tumors;average age 48.6 years) with spinal disorders completed pre/postoperative Maudsley personality inventory (MPI) and postoperative satisfaction. In the spinal tumor group, postoperative improvement was significantly lower with greater satisfaction than the non-spinal group. A greater number of preoperative MPI-positive cases and postoperative negative change of the MPI were seen in the spinal tumor group, while postoperative positive change of the MPI was mostly found in the non-spinal tumor group. Preoperative psychological overlay in the spinal tumor patients was considered to be released after surgery in spite of lower postoperative improvement, and this seemed to lead greater postoperative satisfaction.
