しびれを主訴とした肘窩部ガングリオンによる橈骨神経麻痺の1例

Upper Extremity Numbness Caused by a Ganglion at the Anterior Elbow. Case Report

　上肢のしびれを主訴に受診した場合，ときに漫然と経過をみることがある．症例は38歳，男性．主訴は，突然発症した前腕から手指のしびれであった．某医で頚椎MRIと頭部CTを行ったが，異常がなく当院神経内科を紹介され受診予定であった．受診前日，本人が肘窩部の腫瘤に気付き，当科でガングリオンの診断後手術を行い，速やかに症状が改善した．肘窩部でのガングリオンによる橈骨神経麻痺の報告はほとんど手指伸展障害を主訴としていた．本症例のように，しびれを主訴とした肘窩部ガングリオンの報告は少なかった．

　Compression neuropathy of the radial nerve at the elbow is rare. Radial nerve neuropathy at the elbow mainly causes a motor deficit known as posterior interosseous syndrome. We report a case of upper extremity numbness caused by a ganglion at the anterior elbow. A 38-year-old was with sudden onset of numbness over the upper extremity was found in magnetic resonance imaging of the cervical spine and computed tomography of the brain to have no abnormal lesions. Three weeks after onset, the man noticed a mass at the cubital fossa, leading to a diagnosis of radial nerve compression neuropathy by a ganglion at the anterior elbow. Surgery completely relieved the symptom.
