大後頭孔拡大術における手術操作の安全域―解剖学的検討

Occipital Bone Safety Zone for Foramen Magnum Decompression

　後頭骨の安全域を明らかにするために，日本人成人頭蓋骨標本52体を用いて解剖学的検討を行った．横洞溝，乳突孔，および顆管を同定し，それらの存在する範囲の計測を行った．横洞溝は．外後頭隆起より2.2cm以上尾側には存在せず，乳突孔は正中から3.6cm以内には存在しなかった．顆管と大後頭孔縁からの最短距離は約4mmであった．以上の結果から，後頭骨の安全域は，外後頭隆起より2.2cm以上尾側で，外側3.6cm以内の範囲で，なおかつ，大後頭孔外側縁から4mm以内の範囲であるといえる．

　We conducted an anatomic study to clarify occipital bone safety zone for foramen magnum decompression using 52 dried Japanese skulls for location of the transverse sulcus, condylar canal, and foramen mastoideum. The transverse sulcus was observed within 2.2cm inferior to the external occipital protuberance in all skulls. All foramen mastoideums were observed up to 3.6cm lateral from the midline. The shortest distance from the lateral border of the foramen magnum to the condylar canal was 4mm. We concluded that the occipital bone safety zone was at least 2.2cm inferior to the external occipital protuberance, within 3.6cm lateral from the midline, and within 4mm from the lateral border of the foramen magnum.
