 We conducted an anatomic study to clarify occipital bone safety zone for foramen magnum decompression using 52 dried Japanese skulls for location of the transverse sulcus, condylar canal, and foramen mastoideum. The transverse sulcus was observed within 2.2cm inferior to the external occipital protuberance in all skulls. All foramen mastoideums were observed up to 3.6cm lateral from the midline. The shortest distance from the lateral border of the foramen magnum to the condylar canal was 4mm. We concluded that the occipital bone safety zone was at least 2.2cm inferior to the external occipital protuberance, within 3.6cm lateral from the midline, and within 4mm from the lateral border of the foramen magnum.
