膵頭十二指腸切除術での膵頭神経叢Ⅰ部・Ⅱ部の取り扱い方—その解剖と切離方法

【ポイント】

◆膵癌に対する膵頭十二指腸切除術において神経・線維組織（NFT）構造に基づく切除（NFT- based resection）は，手術時間の短縮，出血量の軽減に導く．

◆膵頭神経叢Ⅰ部・Ⅱ部の正確な領域を術中に確認するのは難しい．確実かつシンプルに膵頭神経叢を切除するため，上腸間膜動脈（SMA）周囲神経叢のoutermost layerであるSMAⅠ・Ⅱ・Ⅲを先行露出する方法が有用である．
