　重症筋無力症（以下MG）治療の第一選択は拡大胸腺摘出術とされている。1975年6月から1999年3月まで，MGに対する当科の胸腺摘出術施行症例は1歳7カ月から82歳までの300例である。眼筋型症例69例，全身型症例231例。胸腺腫合併例は眼筋型症例15例，全身型症例71例の86例で，胸腺腫合併頻度は28.7％となる。これら症例を5年毎，5期に分け，現時点での手術後の生活活動状況，服薬治療の有無などにつき遠隔調査を施行した。効果判定基準は，1）寛解，2）著明改善，3）軽快，4）不変，5）悪化，6）死亡の6段階方式とした。

　胸腺腫非合併例214例中，寛解75例，著明改善56例，軽快55例で86.9％が軽快以上であり，胸腺腫合併例では86例中，寛解14例，著明改善21例，軽快32例で，軽快以上の症例が77.9％を示した。長期遠隔状況をみてもMG治療に対する拡大胸腺摘出術の有用性が示された。

From June 1975 to March 1999, 300 patients of my-asthenia gravis (MG) have undergone thymectomy. Among these patients, 69 cases were classified as ocu-lar type of MG (including 15 cases with thymoma), and 231 were generalized type of MG (including 86 cases with thymoma). The efficacy of the treatment was investigated by evaluating current patients' life ac-tivity, which is classified in 6 groups (Remission, Much Improved, Improved, Unchanged, Worse, and Died).
