　初発時に頭蓋骨病変を伴う肥厚性硬膜炎と考えられた1例を経験した。症例は70歳，男性。左半身痙攣発作にて発症し，画像所見にて右頭頂傍矢状部の浸潤性頭蓋骨病変が認められ，試験切除術を行った。病理組織は骨・硬膜ともに同様な組織変化を示しており，炎症性肉芽腫病変であった。ステロイド治療にて症状・画像所見とも改善した。退院後も痙攣発作はコントロールされていたが，3年後に全身痙攣・右片麻痺にて再発した。MRIにて両側頭頂部と大脳鎌に増強効果のある硬膜肥厚像がみられ，脳血管撮影では上矢状静脈洞閉塞が認められた。主病変は肥厚性硬膜炎であり，再発時の痙攣発作は肥厚性硬膜炎の再燃による上矢状静脈洞閉塞に伴う静脈性脳梗塞によると考えられた。初発時の頭蓋骨病変は，炎症性肉芽腫病変が頭部外傷あるいは上顎洞炎が契機となって，頭頂骨傍正中部のくも膜顆粒小窩部硬膜より発生し，骨破壊性に増殖したことによる変化と考えられた。

We report a case of hypertrophic cranial pachymen-ingitis (HCP) developed skull lesion. A 70-year-old male presented with the symptom of left hemiconvul-sion. MRI revealed that the enhanced intraosseous mass infiltrated into the the dura and brain paren-chyma under the parasagittal region of the right parie-tal bone. Histological examination revealed chronic in-flammation with lymphoplasmacytic infiltrate and fibro-sis of both intraosseous mass and dural invasive le-sion. Steroid therapy resulted in improvement of clini-cal symptoms and enhanced lesion of MRI.
