　無症候性髄膜腫が発見される機会が増えてきているが，その治療や手術適応には多くの問題がある。本例は無症候性の比較的大きな左前頭円蓋部髄膜腫で，客観的な神経心理学的評価を得る目的でWAIS-R（Wechsler Adult Intelligence Scale-Revised）を術前術後に施行した。術前には器質脳疾患を疑わせる所見として，予測された全IQより低値，言語性IQと動作性IQとの乖離，「類似」「積み木模様」問題の低評価点などが見られたが，手術摘出によって消失ないし改善が得られた。今後，髄膜腫に限らず無症候性疾患を手術する場合は，潜在的な神経心理学的異常が明らかになることもあるので，手術適応を決定する意味でも術前に神経心理学的検査が必要と考える。さらに手術前後に神経心理学的検査を行って，手術による神経心理学的影響を評価しておくことは，患者の術後ADLとQOLに重要となる。

The authors reported a case of 39-year-old man with incidentally found meningioma whose postopera-tive WAIS-R scores were improved significantly.

He got a minor head injury run in a car collision and showed no neurological findings on admission. Computed tomography revealed no traumatic abnor-malities but left frontal convexity meningioma. Total removal of the tumor was performed 3 weeks after theadmission. Preoperative verbal IQ, performance IQ, lower full IQ were 105, 95, 100 respectively, and im-proved to 113, 112, 114 postoperatively.
