　慢性硬膜下血腫片側例のr-CBFを的臨床別にheadache，hemiparesis，consciousness disturbance（CD）の3群，正中偏位の程度でmild，moderate，severeの3群に分類し検討した。

　1）慢性硬膜下血腫では血腫側が非血腫側に比べて被殻，視床で脳血流が低下し，前頭葉，後頭葉，小脳では逆に非血腫側で脳血流が低下した。2）臨床症状の重篤な順で脳血流障害が進行した。Headache groupでは非血腫側の後頭葉や前頭葉の皮質の血流低下が見られ，hemiparesis groupではさらに血腫側の被殻，非血腫側の小脳の血流低下が拡大していく傾向が認められた。また，hemiparesis groupでは血腫側と非血腫側の両側性に血流低下を認め，CD groupではさらに顕著となった。3）正中偏位とr-CBFでは，正中偏位が高度になるに従い前頭葉，後頭葉，小脳で非血腫側が血腫側よりr-CBFが低下し，被殻と視床では逆に的血腫側が非血腫側より低下する傾向が顕著となった。

Cerebral blood flow (CBF) measured by ^99m Tc-HMPAO SPECT before operation was studied in 60 patients with unilateral chronic subdural hematoma. The regional CBF was measured in 26 regions of the fronto-occipital 10 cortices, putamen, thalamus and cerebellar hemisphere on both sides.
