　両側性慢性硬膜下血腫34例の脳血流を症状別にheadache， hemiparesis，tetraparesis，consciousnessdisturbance（CD）の4群，脳の正中偏位の程度でnon-shifted，shiftedの2群に分類し検討した。1）両側性慢性硬膜下血腫では，前頭葉，頭頂葉，後頭葉の一部で厚血腫側より薄血腫側の血流が有意に低下し，被殻では逆に薄血腫側より厚血腫側の血流が低下する傾向があった。2）症状別に両側の脳血流を比較するとheadache groupの前頭葉と頭頂葉，後頭葉の一部，CD groupの後頭葉の一部で薄血腫側の脳血流低下を認め，headache groupの視床では逆に厚血腫側で脳血流の低下を認めた。3）CD groupでは両側の全体的な脳血流低下が顕著であった。4）脳の正中偏位と脳血流の関係では，shifted groupの前頭葉，頭頂葉，後頭葉で厚血腫側と比べて薄血腫側で有意に低下していた。non-shifted groupでは両側で有意差はなかった。

Cerebral blood flow (CBF) in 34 patients with bilat-eral chronic subdural hematoma was measured by ^99mTc-HMPAO SPECT before operation. The regional CBF was measured in 26 regions of the 10 cortical re-gions, putamen, thalamus and cerebellar hemisphere on both sides. According to the thickness of subdural hematoma, the thicker hematoma side was measured and examined as the thick hematoma side, and the other side as the thin hematoma side.
