　視覚障害，歩行障害で入院した68歳男性。進行する痴呆とミオクローヌス，脳波上の周期性同期性放電（PSD）の所見などからCreutzfeldt-Jakob病（CJD）と診断した。病初期にアテトーゼ様の運動が観察され，hyperkineticな型の臨床像と考えられた。同時期の画像では，MRIにて尾状核，被殻のT2高信号と軽度萎縮像，SPECTにて前頭葉，側頭葉，一部の頭頂葉，視床に血流低下の所見を認めた。これら画像所見の特徴を検討することは，本症の臨床症状との関連で意義あることと思われた。

A 68-year-old man was hospitalized on 24 June, 1998 because of visual and gait disturbance. A month before admission, he had been aware of blurred or double vision while watching TV. A few days later, he developed dysphagia and clumsiness in the fingers. His gait became unstable and he exhibited restless fin-ger movements. His shoulders and trunk showed tor-sion while walking. On admission, he became disori-ented and showed rigidity in the legs and athetosis in the bilateral fingers. Routine laboratory findings, thy-roid function data, and the serum levels of vitamin B1, B12, Cu, and ceruloplasmin were within the normal ranges.
