(Background & Objective) To evaluate MR findings and ADC maps of Creutzfeldt-Jakob disease.

(Materials & Methods) Routine images and diffusion-weighted images (DWI) were obtained in 4 patients with 1.5T MR scanner. Serial MRI was obtained in 2 patients. MR images were assessed by two neuroradiologists. An apparent diffusion coefficient (ADC) maps were calculated and evaluated statistically.

(Results & Conclusion) T2-weighted images (T2WI) in one patient showed no abnormalities. DWI in all patients showed high intensities in the caudate nuclei and putamen bilaterally. This finding is specific in CJD patients, and DWI is more sensitive than T2WI in CJD lesions, ADC values in the caudate nuclei and putamen were decreased statistically in CJD patients. T1-weighted images in 2 patients of whom MRI was obtained serially showed high signal intensity changes in the globus pallidus bilaterally. DWI is useful for the diagnosis of CJD (especially in the early state).
