　症例は69歳，男性。右の難聴を精査中，incidentalに左前頭葉にcystic massを認め手術となった。肉眼的にはmassと硬膜との癒着は高度であったが，くも膜との境界は明瞭で髄膜腫様であった。病理診断は一層の立方-円柱上皮よりなるepithelial cystであった。Neuroaxisに発生するepithelial cystの起源がectoder-mal, endodermal, mesenchymalのいずれであるかを検索するには電顕や免疫組織学的検討が重要であるが，自験例では明らかにependyma細胞や脈絡叢との連続性は無く，gliaや神経細胞など，いわゆる外胚葉系組織も存在せず，またPASおよびmucicarmine染色にて陽性であったためheterotopicなendodermal cystと診断した。

The patient was a 69-year-old male. During exami-nation of possible deafness, a cystic mass was found in the left frontal lobe, which required surgical inter-vention. Though the mass adhered strongly to the dura mater macroscopically , the boundary between the mass and arachnoid was clear, having a men-ingioma-like appearance. The pathological diagnosis was epithelial cyst consisting of layered cubic to cylin-drical epithelium.
