　振戦を伴うパーキンソニズムで発症し，痴呆と垂直性眼球運動障害が加わり全経過3年で死亡した75歳，男性の1剖検例を経験した。大脳は前頭・頭頂葉萎縮を呈し，皮質病変は中心前回に最も強調されていた。さらに黒質の高度の神経細胞脱落に加え，淡蒼球，視床，中脳被蓋，小脳歯状核にも変性を認めた。Ballooned neuronやタウ陽性構造物は認められなかった。本例はKawasakiらが報告した2例（Acta Neuro-pathol 91：140, 1996）に酷似している。現時点ではfrontotemporal dementia and parkinsonismに最も近縁な疾患と考えられるが，遺伝歴の有無や病変の分布などの点で必ずしも合致せず，今後ひとつの疾患群として捉えていく必要もあると思われた。

We report a 75-year-old man who presented with parkinsonism, dementia and vertical gaze palsy. Postmortem examination revealed frontoparietal atrophy and nigral depigmentation. The temporal lobe was well preserved. Histologically, the affected cerebral cortex showed vacuolation and neuronal loss in layers 2 and 3. The substantia nigra and globus pallidus were also affected. There was gliosis in the thalamic medial nuclei and midbrain teg-mentum. Neither ballooned neurons nor tau-positive, argyrophilic inclusions could be found.
