妊娠中に発症した多発神経炎を伴う麻疹脳炎

A Case of Measles Encephaloneuropathy in a Pregnant Woman

　症例は26歳女性。妊娠中に麻疹に罹患し，第5病日に意識レベルの低下が起こり，髄膜刺激症状，髄液所見，脳波，頭部MRIなどにより，急性麻疹脳炎と診断された。また，深部腱反射の消失や筋電図の異常により，多発神経炎の合併も判明した。これらは免疫学的アレルギー反応によるものと考えられた。早期のステロイドと免疫グロブリン投与により完全寛解した。妊娠中の急性麻疹脳炎の報告は2例みられるが，完治したのは1例のみである。また，ほぼ同時期に中枢および四肢末梢の両方で麻疹による神経障害がみられたという報告も非常に少なく，筋電図所見などを含めた詳細な報告は本邦では初めてである。多発神経炎の合併は急性麻疹脳炎の病態を考える上で重要であると考えた。

We reported a patient with measles ence-phaloneuropathy. A 26-year-old woman in her 15th week of gestation became febrile and developed cutaneous eruption typical of measles on July 1 1997. Five days after appearance of the rash, fetal death was identified and the fetus was removed. Following the operation, she became comatous. Neurological examination revealed neck stiffness, flaccid paralysis of the four limbs, and decreased sweating in the lower limbs. CSF protein was 143mg/dl with cell count of 1365/mm³. Myelin basic protein in CSF was positive.
