A 17 year-old male. The right hemiparesis and hemihypoesthesia appeared at age 12 and were aggravated slowly. Dementia, character and emo-tional change, and abnormal behavior were observed at 15. At age 16, consciousness disturbance and forced laughter appeared. Last 5 month, he was in state of aknetic mutism. All clinical course of patient, died of hemorrhage in the cerebral ventricles at age 17, were 4 year 7 month.

Cerebral arteriographies and pneumoencephalo-grams revealed atrophy of the left cerebral hemi-sphere and intracranial pressure was not increased.

In autopsy, tumor localized in the hypothalamus and wand of 3rd ventricle and invaded diffusely widely white matter from frontal to occipital love and pons.

Histopathological diagnosis was two cell pattern pinealoma. In Japan, 6 cases was reported which similar brain tumor. We would like to emphasize that if we obsearved similar case, we suspect ectopic pinealoma.
