Among the 166 cases of intracranial aneurysm operated by the author intracranially, two cases had large hematoma in the Sylvian fissure. This hematoma have been previously described as ex-ample of intracerebral hematoma produced by ruptured aneurysm. The hematoma of these cases localized within Sylvian fissure and the origin of the aneurysm was from the middle cerebral artery.

The author pointed out the characteristic dis-position of Sylvian fissure in formation of the massive hematoma from the neuroanatomical point of view.

Clinically, one patient showed hemiplegia and aphasia, and other patient showed mental change in the course of recovery of hemiplegia. These symptom has diagnostic value of the Sylvian fissure hematoma in the case of cerebral aneurysm. The mental change has value as symptom caused by compressive effect of hematoma to the frontal operculum and island.

The author also described on the angiographic evidence which may allow differentiation of the hematoma in the Sylvian fissure from other in-tracerebral hematoma.
