Two successful removal cases of brain metastasis following resection of the lower esophageal cancer were reported. The first case was 45-year-old man who suffered from headache and gait disturbance 11 months after the surgery for esophageal cancer following postoperative radiation of 6,000r with Lineac acceleration. The metastatic brain tumor weighning 50 gm of the right parietal region was squamous cell cancer. As a supplementary treated, antineoplastic "Bleomycin" was given intraven-ously, 300 mg in total. This patient is living well and walks by himself at present, 2 months after the brain surgery.

The second case was 44-year-old man, who suf-fered from headache and hemiparesis of left side 5 months after the radical resection of the intra-thoracal esophageal cancer and postoperative radia-tion of 6,000r with tele 60Co. Solitary metastatic brain tumor weighning of 50 gm was removed successfully from the the right motor area by craniotomy. Histological picture was squamous cell cancer. Intravenous administration of Bleo-mycin, 300 mg in total was also added and irradia-tion was done 5,500 r with Lineac accelerator for operated lesion. This patient is living well in satisfactory condition at present after 15 months after the brain surgery.

In these both cases, severe hemiparesis and/or intracranial hypertension were apparently improved or almost disappeared after brain surgery. Modern evolution of esophageal cancer surgery might give us a chance to encounter a brain metastasis, as a result of prolongation of the patients' life.

As a conclusion, we think brain metastasis fol-lowing successful resection of esophageal cancer has to be removed positively, if it is decided that the brain lesion can be removed safely.
