真性多血症における脳障害について

CENTRAL NERVOUS SYSTEM MANIFESTATION OF POLYCYTHEMIA VERA

Ｉ．はじめに

　末梢血液中の血球要素が著しく増加する真性多血症は比較的まれな疾患であるが，欧米では既にいくつかの総括的な研究報告^{2）12）14）}があり，我が国でも最近ではかなりの数の症例報告がみられるようになつた。

　一方1892年Vaquez^13）の初期の報告においても既に本症の重要な合併症の一つとして，中枢神経疾患の出現が知られ，近年では殊に脳血管障害の高頻度の合併が注目されている。しかし本邦における報告は，主として一般内科的立場からのものが多く，本症にみられる脳血管障害に関しては臨床神経学的立場からの詳細な研究報告は極あて少なく，僅かに藤島ら^3），古徳ら^7）のそれがみられるにすぎない。さらに本症にしばしば合併する精神症状に着目した報告は本邦では皆無といつてよい。

Author reported three interesting cases of polycy-themia vera with central nervous manifestation.

Case 1: 60 year-old man, suffered from expres-sive aphasia and right hemiparesis. Angiographi-cally, occlusion of left internal carotid artery was revealed, and there were scarcely noted any colla-teral vessels to the territory of middle cerebral artery on the affected side. there were not foun-ded any arteriosclerotic changes in the carotid artery on angiograms.

Case 2: 36 year-old woman, complained of ver-tigo, headache and insomnia. She had oftan diz-zines and dull ache, when her neck was straind backward. In this case it was dealt with vertebro-basilar insufficiency associated with polycythemia.

Case 3: 30 year-old man who had dapressive mood, headache, weakness and fatigue. In all of them mental depression was most outstanding sym-ptom. This case appeared to be alike to so-called masked depression.

Regarding to the above-mentioned manifestation, author briefly discussed in reference of the Litera-ture.
