  • Look Inside

Fourteen chronic and five acute subdural hema-tomas which were experienced for 14 months after Kobe Children's Hospital was opened, were studied.

The clinical characteristics, diagnosis, neurosurgi-cal treatments and prognosis of the subdural hema-toma in infancy and childhood, these are analysed in this paper.

The important 5 factors of the clinical character-istics are past history of the head injury and the bi th injury (69%), disturbance of the consciousness (54%), convulsion (61%) paresis (70%), and fever (70 %). The retinal bleeding of the chronic subdural hematoma is a few in incidence, and valuable sign, if it is presented.

The symptoms and signs of the chronic subdural hematoma in infancy and childhood were improved especially by the suture widening and this character-istic improvement corresponded to the time of the suture widening. This suture widening did not occur only in the chronic cases (95%), but also in the acute cases (100%), which were very usefull for the early diagnosis and treatments. If it is seen in the plain skull x-ray films, it must be angio-graphically differentiated from the space occupying lesion in the skull.

The neurosurgical treatments of the chronic sub-dural hematoma were, as following, in our cases.

1. frontal tapping, only……………………3 cases

2. burr hole or trepanation,only…………6 cases

3. 1+2……………………………………………2 cases

4. 3+majorcraniotomy…………………………1 case

5. 4+subdural peritoneal shunt……………2 cases

It was thought that its prognosis was very fair ; postoperative convulsion was seen in the 3 cases, pyramidal tract sign in one case and disturbance of the growth in one case. The other cases were normal neurologically, without sequelae.
