両側対称性脳動脈瘤—中大脳動脈瘤の2例

BILATERAL SYMMETRICAL ANEURYSMS AT THE MIDDLE CEREBRAL ARTERY:REPORT OF TWO CASES

はじめに

　脳動脈瘤の約10〜20％を占めるといわれる多発性脳動脈瘤の中には左右全く対称性に発生する特異な症例があり，1955年Bigelow^1）は自験の8例をも含めて62例の左右対称例を，"Symmetrical bilateral intracranialaneurysms"として報告した。それ以来これらの報告は散見されてはいるが当教室においても現在まで両側中大脳動脈および両側内頸動脈後交通動脈分岐部に鏡像にみられた多発性脳動脈瘤7例を経験している。これらの動脈瘤は動脈瘤の発生要因を探索する上に興味ある事実でもあり，またその治療に関しても種々問題が多いと思われる。今回は特にわれわれが経験した両側対称性脳動脈瘤の中で中大脳動脈に発し，一回の手術により同時処置した症例につき報告し，その治療方法および病因論につき多少の文献的考擦を加へ報告する。

Two cases of the symmetrical bilateral intracra-nial aneurysms of the middle cerebral arteries were presented.

These were cured by driect operation at a time. Our policy for the treatment of the intracranial multiple aneurysms is that all aneurysms should be treated by intracranial direct operation at a time as far as possible.

It seems that linear incision on the bilateral temporal region is avilabie for the surgical pro-ceedure of the bilateral middle cerebral artery an-eurysms.

Pathogenesis of the intracranial aneurysms were also discussed on this report.
