中大脳動脈瘤

MIDDLE CEREBRAL ANEURYSM

I．はじめに

　頭蓋内動脈瘤は，最近の脳神経外科の進歩により，直接外科的に，再出血を防止する治療が可能になり，成果を上げてきている。しかし，その治療法は，動脈瘤の局在，動脈瘤頸の状態，患者の全身状態，年齢，血圧および頭蓋内出血発作後の経過時間など，種々の要素により規定され，治療成績も一様ではない。なかんずく，中大脳動脈瘤は，側大脳裂の奥深く存在するものが多く，運動領・感覚領および言語中枢などが近いこと，またparent arteryが中大脳動脈の本幹であることが多く，これを閉塞すれば，重篤な後遺症を残す危険が大きいことなどから，その手術的治療は困難なものと考えられてきた。

　ここで，中大脳動脈瘤をとり上げ，われわれの経験を基に，他の脳動脈瘤と比較しながら，その臨床的特徴および治療法を述べ，あわせて，文献的考察を加えたいと思う。

The middle cerebral aneurysm does not seem to be surgicaly unfavorable lesion. The authors pres-ented this principle based upon experiences with 14 craniotomies for 15 middle cerebral aneurysms. Th-ere has been no mortality. Slight neurological detr-iorations were found in 3 cases after the surgical treatment. Any rebleeding has not occured among 12 followed-up cases. For these aneurysms, coating and wrapping should be considered as well as clip-ping of the aneurysmal neck. The middle cerebral aneurysm may be suspected when subarachnoid hemorr hage occurs with motor disturbance and aphasia if in the dominant hemisphere.

In addition, the characteristics of aneurysms of other locations were also presented in this paper.
