椎骨動脈写上いちじるしい改善をみたWallenberg氏症候群の1例—本邦61症例の概要

A CASE OF WALLENBERG'S SYNDROME SHOWED MARKED CIRCULATORY IMPROVEMENT IN ARTERIOGRAM AND SUMMARY OF 61 CASES IN JAPAN

I．緒言

　本邦におけるWallenberg氏症候群の報告は少なくないが，脳血管撮影の普及した現在といえども，その血管像を検討した症例はまだ僅少であり，さらにその経過を脳血管撮影によつて追求した症例は報告されていない。最近，われわれは椎骨脳底動脈系の循環不全を証明し，臨床症状の好転に伴う循環障害の著明な改善を明らかにし得たWallenberg氏症候群の1症例を経験したので，当該血管について若干の考察を加え，あわせて本邦におけるWallenberg氏症候群の症例を集計し報告する。

In the early morning of four days before-admission, a 47 years old man complained sud-denly nausea, vomitting, dizziness and motor disturbances of left extremities, and hiccough, hoarse voice and difficulty to swallow came out in the afternoon. The patient was thought 1st type of Wallenberg's syndrome because of acute bulbarparlysis and sensory dissociat-ion, and was proved circulatory insufficiency of vertebro-basilar artery and absence of letf posterior inferior cerebellar artery in verteb-ral arteriograms. Disappearing his chief com-plaints, marked circulatory improvement of vertebro-hasilar artery was showed in the arteriogram done 70 days after admission.

Authors summed up 61 cases of Wallen-berg's syndrome had ever reported in Japan and studied age or sex frequency, sides of damage and causes of arterial occlusion etc.
