重症脳外傷の治療としての低体温法の研究

STUDIES OF HYPOTHERMIA IN BRAIN STEM INJURIES

　重症脳外傷の治療は非常に困難な問題であると同時に看過すべからざる重大な問題である。なかんずく脳外傷の変化が直接脳幹に及んだ場合，乃至は脳外傷に続発する諸変化が脳幹症状を次第に発現して来た場合，症状は重篤化しその治療は困難を極めるものである。我々は既に過去数年来脳外傷の病態生理を探究し，殊に脳幹症状に関して追求し，併せてその治療法につき研究を重ねて来た。その途上に於いてこの方面の研究に人工冬眠法が登場したことは重症脳外傷の治療法研究に対する大きな光明であつた。即ち外界からの侵襲に対する生体の反応性を減じて生命の危機を救わんとする人工冬眠法が，重症脳外傷に於ける生命の危険を救うべく残された道であると考えたのである。かくて我々は脳侵襲—特に脳幹侵襲時に於ける人工冬眠法の影響に関して各種の実験的研究を行い，更に進んで臨床的応用を試み好結果を得た。此等の研究成果に関しては既に昭和29年以降各方面の学会に報告し，かつ各学会関係誌に掲載して来た。その一貫せる研究として更に我々は重症脳外傷の治療に低体温法の応用を試みるべく実験的並びに臨床的研究を行つた。

Following a series of studies concerning the influences of artificial hibernation in cases of cerebral injuries, this is to report on the ef-fects of pharmacologic hibernation and hypo-thermia in cases of brain stem injuries from the phase of water content of the brainand E. E.G. as well as the consequences of application of hypothermia in cases of serious external injuries of the brain.

In our experiment, groups of rabbits, their r-brain stems inflicted with injuries by frontal-fan-shape-cuttings, were taken for instances and, when the brains were extracted three hours after injuries and the amount of free water, connected water and total water res-pectively measured at the five parts of the brain, i.e.: 1, r-cerebral grey matter,1, r-cere-bral white matter and brain stem, it has been noticed that the amount of water content is smallest in cases of hypothermia, the cases of pharmacologic hibernation coming next; from which facts it can be noticed that the swelling of brain after brain stem injuries can be prevented by the application of hypo-thermia and pharmacologic hibernation.
