  • Look Inside

A case of cerebral form of paragonimiasis was reported with a few comments on the diagnostic and therapeutic considerations.

Case: The patient is a 9-year-old boy, ad-mitted with left hemiplegia of 1 year and 3 months' duration. Inhistory, he had an attack of severe headache associated with abrupt high fever some three years before. The similar headache had come twice a month since then. Two and ahalt years before admission, he co-mplained nausea, vomiting, headache, fever, disturbance in consciousness and generalized itchy feeling. Recently he had a spell of high fever of 38° to 40°C. On admission the physi cal examination of the chest revealed no par-ticular findings. High-voltage X-ray picture of the chest, however, was suggestive of cy-stlike lesions due to lung flukes. No eggs were demonstrated in the sputum or feces. No eosinophilia was detected in spinal fluids.Intradermal test based on complement-fixing test reaction was strongly positive. In ven-triculogram and cerebral angiogram, multiple granuloma were suspected in right fronto-tem-poral lobe and occipital lobe. By cranioto-my, cystlike lesions of lung flukes were re-moved.
