 A 51-year-old woman was physiologically, serologically and immunologically diagnosed as having SLE (Systemic Lupus Erythematosus). Her physical manifestations improved and her laboratory data including leucopenia returned to within normal range following steroid administration. Her neuropsychiatric manifestations were character change including euphoria, disinhibition and chronic mental deterioration including memory disturbance and intellectual impairment which remained even after steroid administration. Her MRI showed multiple small lesions in the bilateral dense white matter, and a large lesion in the right fronttemporal area. These findings are not compatible with our own previous MRI data in patients with lupus psychosis. Neither are they consistent with other MRI studies. Her serologic viral studies (Enzyme-Linked-Immuno-Sorbent-Assay) showed elevated titers indicating the presence of herpes simplex in CSF. The patient was rediagnosed as having lupus psychosis overlapping herpes simplex encephalitis.
