精神分裂病圏患者に対する役割啓発的接近法の試み—「自己啓発型精神分裂病患者群」と「役割啓発的接近法」の提唱（第2報）

Principles of Liberal Therapeutic Approach: Liberal therapeutic approach toward heuristic type of schizophrenics (2)

　抄録　本論の第1報で，「自己啓発型精神分裂病患者群」の臨床的諸特徴について詳述した。第2報ではこの類型に対する治療的接近法について述べた。留意点は「自己啓発型」の現実適応的判断基準の体得の仕方にある。すなわち，「自分で考え自分が下した判断で集団生活を試行錯誤し，その成否の経験の積み重ねを通じて体得していく」ことを促すことにある。そのために，患者が何らかの集団（たとえばデイケア）に参加するように方向づけ，集団生活場面における出来事を個人面接場面での話題とする。個人面接場面で治療者はほとんどの場合聞き役となることが望ましい。「問題行動」の直後のみは例外的で，患者が素直な感情表現を回避する場合，治療者は指示的に接する方が適当である。そのことにより患者は現実適応的に行動するようになる。以上のような治療的接近法を「役割啓発的接近法」と名付け，その基本的原則について述べた。

　In the first paper on "Liberal TherapeuticApproach toward Heuristic Type of Schizophrenics", the characteristic clinical features of Heuristic Type of Schizophrenics were discussed. In this paper, appropriate therapeutic approach to them is introduced.

　The principle of the therapeutic approach to the Heuristic Type, which we call "The Liberal Therapeutic Approach" is to let the patient think and act independently. The role of the therapist is to guide the patient to various social situations and to promote the heuristic process of the patient.
