 Clomipramine (CI) and desmethylclomipramine (DC) levels in breast milk and plasma were measured in two depressed patients under CI treatment. The concentration of (CI+DC) in milk was comparable to that in plasma in both patients and it reached 1,000ng/ml in one patient who was receiving a daily oral dosage of 50mg of CI medication with intravenous infusions of 100~125mg.

 Since the levels of CI and DC in milk were apparently higher than those reported in other tricyclic antidepressants (TADs), factors that may contribute to this difference were discussed. It was suggested that the previous reports might underestimate the levels of TADs in the breast milk because some of them were based on obscure data obtained by personal communication and others were uncertain in terms of the analytical method.

 It would be advisable to consider the possible adverse effects to breastfed infants when their mothers require TAD therapy.
