 Clinical knowledge regarding suicidal behavior (SB) in youth remains insufficient although such knowledge is indisputably required in psychiatric practice. This study attempted to identify the clinical characteristics of suicidal patients who started exhibiting SB at 20 years or at a younger age (early SB group) by comparing the data on these patients with those from patients who started exhibiting SB after 20 years of age (non-early SB group). The study population comprised 155 suicidal patients who had been consecutively admitted to the Tokyo Metropolitan Matsuzawa Hospital within a 20-month period. In the patients from the early SB group, borderline personality disorder (BPD), anxiety disorders, and abuse were more frequently observed before the age of 17 years. Logistic regression analysis performed to identify the characteristics associated with early SB suggested that BPD (or the pre-manifestation stage of BPD) and a history of physical abuse in childhood or adolescence are related to the early occurrence of SB. These findings have helped increase our ability to perform psychiatric evaluation of suicidal patients.
