パニック障害の認知行動療法中にparoxetineを減量すると知覚変容を伴う精神病症状を呈した1例

A Case with Onset of Psychotic Symptoms with Perceptual Distorsion by Reducing the Dose of Paroxetine during the Course of Cognitive Behavioral Therapy for Panic Disorder

はじめに

　パニック障害に対する認知行動療法(cognitive behavioral therapy；CBT)を施行中，paroxetineの減量に引き続き，知覚変容を伴う幻覚症状を呈した37歳の女性の症例を経験した。症例を呈示し，さらに①paroxetineの減量が知覚変容を伴う幻覚症状の契機となり得るかどうか，②CBT中のSSRI(selective serotonin reuptake inhibitors)の減量がCBTの効果に及ぼす影響について，文献をもとに考察する。なお，本人から症例報告の同意を得た。
