　発症18年の経過で難治性右心不全を呈し腹水管理が困難であった拘束型心筋症（以下RCM）に対し，腹水管理目的で持続性腹膜透析（continuous ambulatory peritoneal　dialysis；CAPD）を導入し，有効であった症例を経験した．症例は43歳，男性．1974年に心電図異常を指摘された．1979年肝腫大，1982年腹水出現．1984年断層心エコー図，CT，カテーテル検査，心筋生検などから特発性拘束型心筋症と診断された．1989年には胸水も出現し，近医にて胸・腹水穿刺を繰り返し受けるようになった．1992年6月，息苦しさなどの自覚症状が増強し，腹水穿刺も1回2,000ml,2週間毎と頻回になったため右心不全治療を目的に当院に入院した．軽度の腎障害の合併があり，大量の利尿薬投与でも十分な利尿が得られなかったため，7月6日，腹水管理を目的としCAPDを導入した．1日1回約300mlの除水を行い，息苦しさ，食欲低下，全身倦怠感などの自覚症状は著明に改善，右房圧も平均24から19mmHgへと改善した．前負荷が著明に増大する本症の腹水管理に対しCAPDは有用な治療法であると考えられた．

We have reported a case of restrictive car-diomyopathy (RCM) with intractable right ventricular failure and uncontrolled ascites despite conservative therapy, effectively treated by CAPD. The case involved a 43-year-old male whose past history includ-ed an ECG abnormality in 1974, hepatomegaly in 1979, and ascites in 1982. RCM was diagnosed by UCG, CT scan, cardiac catheterization, and myocardial biopsy in 1984. Pleural effusion as well as ascites were detected in 1989 and were drained repeatedly with punctures.
