急激な経過で死亡し剖検にて急性孤立性（Fiedler）心筋炎と診断された1例

A Case of Acute Isolated (Fiedler's) Myocarditis Diagnosed by Histopathological Study, with Rapid Unfortunate Course

　症例は65歳の男性．主訴は入院2日前から出現した呼吸困難，前胸部重苦感であった．入院時よりうっ血性心不全症状を呈し，心電図上心房細動と胸部誘導にてQSパターンに近い心筋梗塞様所見並びにST上昇を認めた．胸部X線では著明な心陰影の拡大，肺うっ血，胸水貯留を呈していた．心エコー上，心嚢液は認めなかった．入院時よりCPK, GOT, LDHは軽度上昇傾向を不した．各種ウイルス抗体は全て陰性であった．入院後2日目に突然心停止，呼吸停止となり死亡した．剖検にて心臓は著明に肥大し（心重量690g），鏡検上心全体にび漫性の細胞浸潤を認めたが，心内膜，心外膜には炎症性変化が認められなかった．またgiant cellを伴う肉芽腫性浸潤が認められた．

　冠動脈の閉塞所見並びに心筋梗塞所見は認められなかった．本症は臨床的には劇症型の急性ウイルス性心筋炎とも考えられるが病理学的にはFiedlerが提唱した急性孤立性心筋炎とすべきであり，本邦においては65歳以上の報告例は稀であるため報告した．

A 65 year-old-man was admitted to our hospital com-plaining of orthopnea and precordial oppressive feeling. Chest roentgenogram revealed congestive heart failure. Electrocardiogram revealed acute myocardial infarc-tion-like pattern. Serum enzymes (CPK, GOT, LDH) were slightly elevated, but serum antiviral antibodies were not elevated. Echocardiogram showed servere symmetrical left ventricular (LV) hypertrophy, but there was no abnormality of LV wall motion.

He died of progressive heart failure and ventricular fibrillation on the second hospital day. A necropsy was performed within one hour of death.
