急性ウイルス性心筋炎として発症後，心不全を繰返し剖検にて慢性心筋炎と診断しえた1例

An Autopsy Case of Chronic Myocarditis, which Occurred as Acute Viral Myocarditis and Relapsed into Heart Failure

　症例は40歳，女性．1988年12月20日完全房室ブロックを来し近医入院．その後心不全，心室性頻拍が出現し，1989年1月6日当センターに転院となった．入院時検査結果から急性ウイルス性心筋炎と診断した．入院後カテコラミン，大動脈内バルーンパンピング，免疫抑制療法により心機能は一時改善したが，その後心不全のため入退院を繰り返し，1990年9月2日死亡した．1回目入院時の心エコー図では心室中隔の壁運動は低下していたが，左室後壁の壁運動は保たれていた．また，発症4カ月後の心筋生検では心筋の錯綜配列，線維化を認めたが，炎症所見は認めなかった．しかし，1990年4月25日5回目の入院時の心エコー図では左室壁運動のびまん性の低下に加え右室の著明な拡大を認め拡張型心筋症様病態を呈した．剖検では数カ所に炎症細胞の集簇を認め，慢性心筋炎と診断した．

A 40-year-old woman was transferred to our institu tion from a local emergency room because of complete atrioventricular block followed by heart failure and ventricular tachycardia. As her viral titer was elevated, we diagnosed her as having acute viral myocarditis. Her heart failure was improved by initiation of cate-cholamines, insertion of IABP and immunosuppressive therapy. Since then, after experiencing several relapses of heart failure, she died 21 months after her first admission.
