大血管転位に対するJatene手術後のタリウム心筋イメージング所見

Thallium-201 Myocardial Imaging in Case Involving Transposition of the Great Arteries after Arterial Switch Operation

　心臓外科手術の進歩に伴い，大血管転位に対するJatene手術の症例数は増加しつつある．本手術の術後遠隔期の問題に，肺動脈狭窄の発生や，冠動脈の移植に関連すると考えられる術後の心筋障害などがある．本稿では，Jatene手術後の大血管転位39例，両大血管右室起始1例の計40例に対しタリウム心筋イメージングを施行し，術後の肺動脈狭窄および心筋障害の評価について検討した．術後の有意肺動脈狭窄は14例に認められた．術後の肺動脈狭窄は肺動脈弁上狭窄であり，これは術後時間の経過とともに進行する例が存在し注意を要すると考えられた．術後の心筋障害例は3例に認められ，冠動脈閉塞は2例に認められたが，1例には認められなかった．後者については，空気塞栓がその原因として想像された．以上のごとくタリウム心筋イメージングは非侵襲的なJatene手術後の経過観察に有用であった．

Forty patients, including 39 with complete transposi-tion of the great arteries and one with double outlet right ventricle, after arterial switch operation were studied with thallium myocardial imaging (TMI). In patients who had post-operative cardiac catheteriza-tion, the TMI findings were compared to those of cath-eterization studies. The right ventricular to left ventricular up-take ratio (RV/LV ratio) shown by TMI correlated well with the right-to-left ventricular systolic pressure ratio.
