大動脈憩室を伴った鏡像的分枝の右側大動脈弓の1例

A case of mirror image type right aortic arch with aortic diverticulum

　右側大動脈弓は，心疾患の合併あるいは血管輪の形成により気管・食道の圧迫をきたさないかぎり，精査の対象となることは少ない，73歳まで無症状に経過し，当科入院を機に，静注法DSAと逆行性大動脈造影にて明らかにしえた，大動脈憩室を伴った鏡像的分枝を有する右側大動脈弓の1症例を，文献的考察を加えて報告する。

A case of mirror image type right aortic arch with aortic diverticulum was reported. A 73-year-old woman was admitted for treatment of recurrence of Paget's disease of the vulva, and right aortic arch was detected incidentally on chest X-ray film. Digital subtraction angiography by intravenous in-jection (IVDSA) revealed a right aortic arch and aortic diverticulum. And thoracic aortography con-firmed the brachiocephalic vessels.

This type of aortic arch anomaly without congeni-tal cardiac abnormalities, has been reported in only a few instances in Japan. IVDSA is excellent modal-ity for screening of right aortic arch.
