A technique to perform angioplasty of the left in-ternal mammary artery coronary graft utilizing brachial approach is described. A 60-year-old man underwent angioplasty for the severe stenosis at the site of anastomosis of left internal mammary artery and left anterior descending coronary artery.Utilizing the ipsilateral brachial artery permitted cannulation of the internal mammary artery with guiding catheter for renal angioplasty. The coronay dilation catheter was passed through the guiding catheter into the internal mammary artery with the use of guide wire. The coronary dilation catheter was placed to transverse the stenosis, A few iofla-tions to a maximum inflation pressure of 12 atomos-pheres and maximum duration of 30 seconds success-fuly dilated the stenosis. Angiograms after angio-plasty revealed no residual narrowing.
