大動脈炎症候群に合併した左冠動脈入口部狭窄に対する一手術例

A case of aortitis syndrome with left coronary ostial stenosis treated by aortocoronary bypass grafting

　大動脈炎症候群は若年婦人に好発し，大動脈とその主要分枝，肺動脈などに狭窄を起こして多彩な臨床症状を呈する特異な疾患である。冠動脈病変の合併は比較的稀であるが，最近冠動脈直達手術の対象として注目されつつある。著者らは大動脈炎症候群に合併した左冠動脈入口部狭窄の1例を経験し，大動脈—冠動脈バイパス術（以下CABG）を行ったので若干の考察を加えて報告する。

A case of 20 year-old female who was suffered from unstable angina due to aortitis syndrome is presented.

The coronary arteriogram showed the left coronary ostial stenosis more than 90 percent with the visualiza-tion of the left anterior descending artery (LAD) through collateral vessels from right coronary artery.

After successful sequential aorto-coronary bypass grafting to LAD and the first obtuse marginal branch, the patient has been free from anginal pain.

Diffuse fibrosis and thickening of the aortic wall was disclosed by the histological examination.
