心筋電極によるA-V sequential pacingを施行した完全房室ブロックを伴う修正大血管転換症の一成人例

An adult case of corrected transposition of the great arteries complicated atrio-ventricular block treated with A-V sequential pacing using myocardial electrode

　修正大血管転換症（corrected transposition of the greatarteries以下C・TGA）は，大多数の例に心室中隔欠損症，心房中隔欠損症．肺動脈弁狭窄症等の心奇形を合併するため予後不良例が多いので，成人に至って初めて発見される例は比較的まれとされている。一方，本症はしばしば房室伝導障害を合併するといわれている^1）。

　最近，我々は，完全房室ブロックを伴うC・TGAの1成人例を経験し，そのブロック部位を確認し、心筋電極によるA-V sequential pacing挿入により良好な経過が得られたので報告する。

An adult case of corrected transposition cf the great arteries associated with complete atrioventricular block treated vrith A-V suquential pacing was reported.

A 28-year-old man was found, for the first time, to have this abnormality.

The 3rd degree A-H block and prolonged ventricular recovery time were documented by electrophysiologic investigation. The A-V sequential pacing using myo-cardial electrode was successfully used, as endocardial electrode into the anatomical left ventricle could not he stabilized.
